This proposal isn’t controversial, it gets heavy support.
This is irrelevant if and when Italy is going to vote in favour of Mercosur, what it reportedly will. It would be enough for the necessary majority vote and the deal could be signed on Jan 12.

Why?
From the article:
Unfortunately, the outcome in this negotiation is that although the EU has agreed to a number of additional measures, they are not sufficient to satisfy our citizens. “So our position stands. We will vote against the agreement.”
That doesn’t answer the question, it’s just unspecific. Which measures were added, why are they not sufficient, what would satisfy the Irish? Those details are needed to sufficiently answer the question, so: why?
